Beginning October 15, 2021, AOC will resume completion of all medical records requests and FMLA/Short-Term Disability Paperwork.

 

Medical Records Requests

AOC strives to provide quality patient care and will process your medical records request in a timely fashion. Please fax requests for medical records to 251-281-1053 or email to ROI@alortho.com.

Your request will begin processing once the consent form has been completed and submitted.  Please allow 5-7 business days.  If you have questions or concerns, please email ROI@alortho.com or call 251-410-3652.

 

FMLA / Short-Term Disability Paperwork

We are committed to you and your health and are here to assist in the completion of FMLA/STD paperwork.  Please fax your form(s) to 251-410-4154 or email as an attachment to forms@alortho.com.  Please make sure that you have completed the patient section of your form prior to sending it to avoid any delays in completion. If you have any questions regarding your form, please email forms@alortho.com or you may call 251-410-3810.

Once we have received your form you will receive an email (if your form was submitted via email) with a payment invoice.  Once payment has been processed, your form will be completed within 7-10 business days and returned to you by the return route you have selected.

If your form is submitted via fax, please make sure you list a contact telephone number and/or email address and you will be contacted with payment instructions.

There is a $ 26.50 fee for most form completion.
